Job Title: Home Health Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in Warrensville Heights, Ohio.

Job Summary:

We are looking for a compassionate and dedicated Physical Therapist to provide home care therapy to individuals in need. As a Physical Therapist with VNA Health Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Conduct initial assessments to determine patient treatment needs and develop personalized plans of care.
  • Provide direct physical therapy services, including performance of physical therapy procedures, instruction of patients and caregivers, and recommendations for equipment or adaptation of home environments.
  • Re-evaluate and revise plans of care as needed with physician approval.
  • Collaborate with other healthcare professionals to ensure comprehensive care and make referrals as necessary.
  • Maintain accurate and timely documentation of patient progress and treatment plans.
Requirements:
  • Bachelor's Degree in Physical Therapy preferred.
  • One year experience in a supervised healthcare setting.
  • Strong planning and organizational skills, with the ability to work independently and as part of a team.
  • Demonstrated ability to maintain strict confidentiality and handle sensitive patient information.
  • Licensed Physical Therapist by the State of Ohio and CPR certification required.
