Job Title: Home Health Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in Warrensville Heights, Ohio.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide home care therapy to individuals in need of rehabilitation and pain relief. As a Physical Therapist with VNA Health Group, you will be responsible for developing and implementing individualized treatment plans, conducting assessments, and providing education to patients and caregivers.

Responsibilities:
  • Conduct initial assessments to determine patient treatment needs
  • Develop and implement individualized treatment plans with measurable goals and specific discipline-related treatments
  • Provide direct physical therapy services, including performance of physical therapy procedures, instruction of patients and caregivers, and recommendations for equipment or adaptation of home environment
  • Re-evaluate and revise treatment plans as needed with physician approval
  • Facilitate total care services to patients by making appropriate referrals to other disciplines and obtaining physician orders or consulting with case managers
  • Submit all documents in a timely manner
  • Obtain physician approval to modify current treatment plans or treatment programs
  • Perform other related duties as assigned
Qualifications:
  • Bachelor's Degree in Physical Therapy preferred
  • One year experience in a supervised healthcare setting
  • Experience working as part of a team and working effectively with diverse cultural, social, and ethnic backgrounds
  • Strong planning and organizational skills
  • Demonstrated ability to work well under pressure, within strict time parameters, and with a strong attention to detail
  • Ability to work independently with minimal supervision
  • Demonstrated ability to maintain strict confidentiality
