Job Overview

The Licensed Practical Nurse (LPN) plays a crucial role within the healthcare team, utilizing sound nursing judgment to provide exceptional care.

Key Responsibilities

The LPN is responsible for delivering both direct and indirect patient care for a designated group of patients, under the guidance of a Registered Nurse (RN) and in accordance with the established care plan derived from physician orders and RN assessments.

  • Provides comprehensive nursing care that encompasses physical assistance, emotional support, and education for patients and their families.
  • Assists the RN in gathering essential patient health data during admission through focused assessments that consider the physical, psychosocial, and cultural needs of each patient.
  • Collaborates with the RN to implement and evaluate individualized care plans tailored to the patient's age and specific needs.
  • Participates in the reassessment of patient conditions and takes appropriate actions as necessary.
  • Engages patients and their families in the care process, ensuring effective communication and understanding.
Documentation and Compliance

Maintains accurate and timely documentation of patient care activities using electronic medical records, ensuring compliance with hospital policies and professional standards.

Patient Monitoring

Observes and reports significant changes in patients' conditions to the appropriate healthcare professionals, including:

  • Physical and mental health status.
  • Behavioral changes and interpersonal dynamics.
Nursing Functions

Performs nursing duties as outlined by Pennsylvania regulations, including:

  • Initiating and managing IV therapy.
  • Supporting patient rehabilitation in line with medical care plans.
  • Encouraging self-care and independence in daily activities.
Education and Advocacy

Facilitates patient education on medication management, pain control, and available resources, ensuring patients understand their responsibilities in their care.

Assists the RN in assessing educational needs and barriers, adapting teaching methods to suit individual patient requirements.

Collaboration and Communication

Maintains effective communication with physicians and other healthcare providers to achieve optimal patient outcomes and ensure seamless care transitions.

Participates in interdisciplinary meetings and provides relevant information during shift reports and patient transfers.

Quality Improvement

Engages in performance improvement initiatives aimed at enhancing patient care quality and service delivery.

Professional Development

Demonstrates leadership and accountability in nursing practice, acting as a mentor to new staff and students.

Required Qualifications
  • Graduation from an accredited Practical Nursing program.
  • Strong organizational, leadership, and critical thinking skills.
  • Effective communication abilities for diverse audiences.
Licensure and Certifications

Current licensure as a Licensed Practical Nurse in Pennsylvania and certification in Basic Life Support (BLS) are mandatory.
