Responsibilities of a School-Based Practical Nurse (LPN):
- Deliver in-home direct patient care in accordance with the provider's prescribed care plan while closely monitoring recovery progress.
- Evaluate and document patient conditions, reporting any changes as necessary (vital signs, blood/stool/urine, glucose, etc.)
- Administer medications, insulin, and IV fluids while maintaining thorough documentation.
- Inspect and manage wounds, change dressings, and assist with personal hygiene needs.
- Assess patients for additional injuries, infections, bedsores, etc., and provide appropriate treatments.
- Collaborate with healthcare providers, aides, and allied health professionals to ensure optimal patient outcomes.
Qualifications for a School-Based Practical Nurse (LPN):
- Licensure: Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in the relevant state
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to and from care sites
- Experience with trach and/or ventilator care preferred, but not required
