Care Navigator Job Summary

We are seeking a highly motivated and compassionate Care Navigator to join our team at The Institute for Family Health. As a Care Navigator, you will play a vital role in ensuring the execution of service plans for patients, promoting healthy living, and providing social support.

Key Responsibilities:
  • Assist patients in addressing personal and environmental difficulties that may negatively impact treatment adherence
  • Accompany patients to medical appointments and provide transportation support as needed
  • Conduct home visits and community outreach to engage patients and promote healthy living
  • Provide case management to address patient needs related to treatment adherence, including transportation and childcare support
  • Collaborate with interdisciplinary teams to ensure evidence-based care and patient engagement
Requirements:
  • HS diploma or GED required; Bachelor's degree preferred
  • Knowledge of healthcare programs and services
  • Excellent interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Bi-lingual, Spanish preferred
