Job Summary

This position is responsible for conducting in-home visits to monitor and report on the quality of services provided by Home Care Aides. The successful candidate will ensure that existing care plans are adequate to meet client needs.

Key Responsibilities
  • Client Monitoring: Conduct regular in-home visits to monitor the condition and performance of duties by Home Care Aides.
  • Regulatory Compliance: Maintain all company and Department on Aging rules, regulations, and standards.
  • Client Onboarding: Conduct home visits to new clients to welcome and review the Welcome Packet.
  • Evaluation and Reporting: Prepare and complete accurate evaluation reports and case notes as needed.
  • Scheduling and Attendance: Maintain schedules to assure timely completion of all assigned home visits and attend weekly branch meetings.
  • Confidentiality and Compliance: Maintain a high degree of confidentiality at all times due to access to sensitive information and follow all Medicare, Medicaid, and HIPAA regulations and requirements.

  • Customer Service: Maintain positive internal and external customer service relationships.
  • Communication: Maintain open lines of communication and plan and organize work effectively.
  • Productivity and Teamwork: Meet all productivity requirements, demonstrate team behavior, and promote a team-oriented environment.
  • Continuous Quality Improvement: Actively participate in Continuous Quality Improvement and represent the organization professionally at all times.
Requirements and Competencies
  • Experience: Minimum of 3 years' experience working in the field of senior services or related social service.
  • Certifications and Training: CNA certificate and license preferred; or similar training.
  • Interpersonal and Communication Skills: Strong interpersonal and communication skills.
  • Computer Skills: Computer experience – Microsoft Word and Excel.
  • Transportation: Must have reliable transportation, valid driver's license, and insurance for travel within the branch service areas.
  • Language Skills: Bilingual preferred.
