Job Title: Heart Transplant Coordinator

As a Heart Transplant Coordinator, you will play a vital role in the transplant team, working closely with top clinical, technical, and business professionals across all disciplines.

Key Responsibilities:

  • Provide direct care and coordinate care for patients with end-stage organ failure referred to the Transplant Program.
  • Assess and evaluate patients and families, developing a plan of care in collaboration with the transplant team.
  • Develop a written, continuous record for each patient, implementing identified medical and nursing interventions, and evaluating outcomes.
  • Serve as an integral member of the transplant team, ensuring continuity of care throughout the transplant process.

Requirements:

  • Licensed to practice professional nursing in the Commonwealth of Pennsylvania, R.N.
  • BSN Graduate of an NLN-approved school of professional nursing.
  • Four or more years' experience as a professional nurse, with prior experience in transplant not required but experience in cardiology preferred.
