We are seeking a skilled and compassionate Post-Heart Transplant Coordinator to join our team. As a key member of our transplant program, you will play a vital role in ensuring the seamless coordination of care for patients undergoing heart transplantation.

Key Responsibilities
  • Provide direct care and coordinate care for patients with end-stage organ failure referred to the Transplant Program
  • Assess and evaluate patients and families, developing a plan of care in collaboration with the transplant team
  • Develop a written, continuous record for each patient, implementing identified medical and nursing interventions, and evaluating outcomes
  • Serve as an integral member of the transplant team, ensuring continuity of care throughout the transplant process
  • Maintain competencies for the care of transplant patients and demonstrate the skills and knowledge to provide ongoing, individualized care
Requirements
  • Licensed to practice professional nursing in the Commonwealth of Pennsylvania, R.N.
  • BSN Graduate of an NLN-approved school of professional nursing
  • Four or more years' experience as a professional nurse
  • Prior experience in transplant not required, but experience in cardiology is preferred
