Job Summary

We are seeking a highly skilled and dedicated Lung Transplant Coordinator to join our team at Jefferson Health. As a key member of our transplant program, you will play a critical role in ensuring the smooth coordination of lung transplant services, from evaluation to post-transplant care.

Responsibilities
  • Coordinate the evaluation and management of potential lung transplant candidates, including waitlist management and follow-up care.
  • Maintain adherence to regulatory requirements, including UNOS, CMS, and Joint Commission standards.
  • Collaborate with multidisciplinary teams, including medical and surgical teams, to ensure seamless care coordination.
  • Support outpatient transplant clinics, round on inpatients, and coordinate follow-up care for transplant patients.
  • Activate and maintain program protocols for patients on the transplant waitlist.
  • Coordinate the admission process for transplant patients and maintain regulatory requirements.
  • Provide post-operative teaching and continuous education to post-transplant recipients.
Requirements
  • Bachelor's of Science in Nursing required.
  • Previous critical care and/or CICU/CV surgical ICU experience preferred.
