Care Manager

We are seeking a skilled Care Manager to join our team at Blue Shield of California. As a key member of our Population Health Management Medi-Cal Team, you will play a critical role in supporting the mission of our department to provide high-quality care to our patients.
Key Responsibilities- Develop and implement individualized care plans to promote quality of care and cost-effective healthcare services.
- Coordinate care with specialists, community resources, and other stakeholders to ensure seamless transitions and optimal outcomes.
- Assess member health behaviors, cultural influences, and client belief/value systems to identify potential barriers and develop targeted interventions.
- Collaborate with department leadership to identify program development opportunities and contribute to continuous improvement initiatives.
- Mentor case managers with complex cases to promote quality of care and cost-effective healthcare services.
- Perform precepting for new hires or those who are not meeting expectations.
To be successful in this role, you will need to have a strong understanding of clinical practice guidelines, healthcare policy, and population health management principles. You will also need to possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
