**Key Responsibilities:**

* Provide personal care and incidental activities of daily living for patients of all ages
* Perform care and delegated tasks as directed by the nursing plan of care and/or supervisor
* Utilize infection control measures and observe and report patient conditions to the appropriate supervisor
* Work in a team environment to effectively deliver care and assistance to patients and staff
* Assist with ambulation, transfers, and range of motion exercises
* Perform patient-related procedures such as vital sign measurements and blood glucose monitoring

**Requirements:**

* Must be 18 years of age
* High School diploma or equivalent and successful completion of Nursing Assistant course and Nurse Assistant certification as required by the state in which employed
* 1 year experience in comparable job classification and/or as required by the contracted facility
* Home Care or Hospice experience preferred
* Proof of COVID Vaccine
