Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Connecticut Institute for Communities, Inc. as a Patient Care Specialist. As a key member of our healthcare team, you will work closely with doctors, nurse practitioners, and other healthcare professionals to provide high-quality patient care.

Responsibilities
  • Collect and document patient information to inform assessment and care planning.
  • Administer medications and treatments as prescribed by healthcare providers, ensuring patient safety and well-being.
  • Monitor and record vital signs, medical history, and symptoms, providing accurate and timely documentation.
  • Assist with physical exams and medical procedures as needed, adhering to licensure requirements.
  • Provide education to patients and families on healthcare topics, promoting health literacy and empowerment.
  • Collaborate with school nurses, WIC colleagues, DCF, and other medical departments as needed, ensuring seamless care coordination.
  • Triage patient calls, using established protocols to determine appropriate care, including at-home management or medical visits.
  • Provide patient and family education on after-hours care, triage, and referral services, as well as disease states.
  • Communicate with specialists to obtain letters and results, and facilitate home therapies as determined by a provider.
Requirements
  • High School Diploma or equivalent.
  • Current LPN license in the state of Connecticut.
  • Previous experience in pediatrics preferred.
  • Strong communication and interpersonal skills, with bilingual abilities strongly encouraged.
  • Ability to work in a fast-paced environment, with attention to detail and multitasking skills.
  • Understanding of medical terminology and nursing protocols.
  • Experience with electronic medical records, with eClinical Works preferred.
