Role Summary

Nuvance Health operates a comprehensive network of hospitals and outpatient facilities, including multiple locations in Connecticut and New York, offering a wide range of primary and specialized medical services.

Our non-acute care services are provided through various affiliates, including rehabilitation and skilled nursing facilities, as well as home care services.


Job Summary:


The Licensed Practical Nurse (LPN) will deliver both direct and indirect patient care while collaborating within a team-oriented environment under the supervision of a Registered Nurse (RN) and in partnership with Patient Care Technicians (PCTs). This role is essential in addressing the current demands for patient care by providing additional support in a challenging healthcare market.



Qualifications:
LPN licensure

Control Hours: 36.00

Work Shift: 3:00 PM - 3:30 AM; alternating weekends

Key Responsibilities:
1. Deliver direct patient care within the LPN scope of practice and established nursing protocols - 40%
2. Document patient information in the Electronic Medical Record (EMR) under RN supervision, including vital signs, wound care, medication administration, and patient admissions - 15%
3. Collaborate with the healthcare team to ensure a safe and therapeutic environment for patients - 20%
4. Verify, prepare, and administer medications following safe practices - 10%
5. Conduct standard diagnostic procedures such as EKGs, phlebotomy, and bladder scans as needed - 5%
6. Assist in planning, implementing, and evaluating nursing care for patients - 10%
7. Apply High Reliability Organization (HRO) principles in daily tasks
8. Integrate the mission and values of Nuvance Health into daily responsibilities
9. Utilize the Relationship Model of Care to enhance patient experiences through effective communication and empathy
10. Participate in interdisciplinary meetings - 2%
11. Engage in team-building activities that support patient care, staff education, and personal development - 1%
12. Maintain clinical competencies to support evidence-based practices - 5%
13. Attend all mandatory and continuing education sessions - 1%

Additional Information:
1. Medical knowledge and understanding of infection control practices
2. Strong critical thinking abilities
3. Preferred experience in a clinical environment
4. Completion of an approved medication administration course and 40 hours of supervised practice
5. Proficiency in EMR documentation systems
6. Familiarity with state nursing practice regulations

Educational Requirements:
1. Completion of a state-approved LPN program
2. Valid LPN license (NCLEX-PN exam passed)
3. BLS certification
4. 1-2 years of recent nursing experience

Work Environment:
1. Essential manual skills and dexterity required
2. Ability to exert significant physical effort, potentially lifting up to 50 lbs.
3. Exposure to various occupational risks
4. Frequent exposure to dirt, odors, noise, and bodily fluids.
