Essential Qualifications

  • A degree from an accredited four-year institution in social work, sociology, psychology, nursing, gerontology, or a related field; AND
  • A minimum of two years of experience in social services, including client interviews, needs assessments, or counseling.

Key Responsibilities

The following responsibilities are indicative and not exhaustive. Depending on the area of responsibility, the incumbent may perform one or more of the activities described below.

  • Evaluates a wide range of social, economic, legal, environmental, occupational, physical, and mental health information obtained through detailed client interviews, observations, research, and investigations; to assess client needs, determine eligibility for services, identify underlying issues, and develop a case management plan aimed at achieving client self-sufficiency across various programs.
  • Oversees the work of case managers, interns, or community volunteers.
  • Provides counseling to individuals, families, and groups; may engage in crisis intervention; advocates for clients; and collaborates with community service organizations to deliver necessary information and services to address diverse socio-economic needs.
  • Conducts outreach and coordinates service activities.
  • Prepares detailed client social histories, program policies, handbooks, case management plans, reports, and other relevant documentation to ensure compliance with local, state, and federal guidelines.
  • Participates in community meetings with stakeholders and service providers, representing the agency.
  • May conduct surveys, research, and develop programs; plans, coordinates, facilitates, and evaluates training sessions and workshops to enhance client capabilities and promote self-sufficiency.
  • Assists in quality improvement initiatives, including ongoing case record reviews, data collection, performance outcome analysis, and audit preparation.
  • Provides client assessments and related information to public officials and collaborates with social service agencies, psychologists, law enforcement, attorneys, and judges to address client needs.
  • Applies principles of family-centered care while managing assigned cases.
  • Conducts quality control functions, auditing case actions, reviewing expenditures, and ensuring documentation accuracy.
  • Facilitates staff training and orientation programs.
  • Performs other related duties as assigned.

Job Requirements

  • Knowledge of social services program delivery theories, techniques, and case management procedures.
  • Familiarity with applicable federal, state, county, and community social service programs, including eligibility criteria and application processes.
  • Proficient in interviewing clients to identify personal, legal, social, economic, and health needs.
  • Skilled in counseling individuals towards achieving self-sufficiency.
  • Competent in applying crisis intervention techniques.
  • Ability to develop effective work procedures and standards.
  • Capacity to collaborate effectively with others.
  • Proficient in planning, organizing, evaluating, and supervising the work of others.
  • Ability to collect, organize, and analyze data to draw logical conclusions.
  • Strong communication skills, both verbal and written.

Physical Demands

  • Must be able to move intermittently throughout the day, utilizing proper body mechanics.
  • Must possess the ability to observe, listen, and communicate effectively.
  • Must work independently, demonstrating flexibility, integrity, and the ability to collaborate with residents, personnel, and external agencies.
  • Must be able to relate to and work with individuals who may be ill, disabled, elderly, emotionally distressed, or at times, hostile.

Work Environment

  • Sedentary Work - Involves exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to move objects. Primarily involves sitting, with walking and standing required only occasionally.

Emergency Management Duties

In the event of an emergency, employees may be required to fulfill duties as assigned by their department or relevant emergency management authorities.
