Job Description
The Registered Nurse will be responsible for providing care and services for a group of patients, visiting patients in their homes, skilled nursing facilities, or assisted living facilities. As a contributing member of the interdisciplinary team, the RN will provide expertise on psych-social assessments, interventions, and knowledge of community sources. This role can work in Case Management, Admissions, or Resource roles.

Requirements
To be successful in this position, you should have:
- Current Registered Nurse licensure (in good standing) in the state(s) of practice
- Strong communication skills to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Ability to work independently and manage multiple tasks, including phone calls, emails, documentation, meetings, and travel across the service area
- Time management skills to prioritize visits, documentation, and meetings
- Valid driver's license, reliable transportation, and auto insurance
