Job Description

As a Registered Nurse (RN), you will be responsible for delivering compassionate care and services to a group of patients in various settings, including private homes, skilled nursing facilities, and assisted living facilities. You will be a valuable member of our interdisciplinary team, providing expertise in psych-social assessments, interventions, and knowledge of community resources.

Responsibilities

Your duties will include:

  • Visiting patients in their residences, conducting assessments, and developing care plans
  • Collaborating with healthcare professionals, social workers, chaplains, volunteers, and CNA leadership to ensure comprehensive patient care
  • Managing patient records, documentation, and communication effectively
  • Utilizing electronic medical records systems to maintain accurate and up-to-date patient information
Requirements

To be successful in this role, you will need:

  • A current Registered Nurse (RN) licensure in good standing in the state(s) of practice
  • Excellent communication and interpersonal skills, with the ability to work effectively within a team environment
  • Strong time management skills, with the ability to prioritize tasks and manage multiple responsibilities
  • Reliable transportation, a valid driver's license, and auto insurance to travel across the service area
