Job Description

The Care Coordinator will be responsible for providing expert care and services to a group of patients, visiting them in their homes, skilled nursing facilities, and assisted living facilities. This role is a vital part of our interdisciplinary team, requiring expertise in psych-social assessments, interventions, and knowledge of community resources. The Care Coordinator will work in either a Case Management, Admissions, or Resource role.

Requirements

To be successful in this role, you will need:

  • To hold current Registered Nurse (RN) licensure in good standing in the state(s) of practice;
  • To possess excellent teamwork skills, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership;
  • To work closely with patients on a one-on-one level while receiving strong support from a patient care team;
  • To have the ability to work independently, managing phone, email, and written communication, meetings with healthcare professionals, and an understanding of hospice rules and regulations;
  • To possess strong time management skills, prioritizing and triaging multiple concerns;
  • To be willing to drive with reliable transportation, a valid driver's license, and auto insurance, to travel across the service area;
