Job Description

The Care Specialist - End-of-Life Patient Support will provide care and services to a group of patients, visiting them at their residences, which may include private homes, Skilled Nursing Facilities, and assisted living facilities. As a contributing member of the interdisciplinary team, this role requires expertise in psych-social assessments, interventions, and knowledge of community resources. The Care Specialist will work in either Case Management, Admissions, or Resource roles.

Required Skills and Qualifications

  • Emphasis on Spanish-speaking candidates
  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
  • Ability to work collaboratively within a team environment, interacting daily with nurses, social workers, chaplains, volunteers, and CNA leadership
  • Strong communication skills, including phone, email, and written correspondence, as well as meetings with healthcare professionals
  • Familiarity with hospice rules and regulations
  • Effective time management skills to prioritize multiple concerns and tasks
  • Ability to learn an electronic medical records system
  • Reliable transportation, valid driver's license, and auto insurance for travel across the service area
