Job Description

Job Overview

As a Bilingual Case Manager, you will play a critical role in delivering social service programs and promoting self-sufficiency among eligible clients. Your primary responsibility will be to collect and analyze socio-economic information, assess client needs, and develop comprehensive plans to address their challenges.

Key Responsibilities

  • Analyze client information to identify deficiencies in personal, legal, social, economic, and health needs.
  • Develop and implement case management plans to promote client self-sufficiency.
  • Supervise case managers, interns, or volunteers from local community organizations.
  • Counsel individuals, families, and groups to help them attain self-sufficiency.
  • Collaborate with community service agencies to provide information and services necessary to meet various socioeconomic needs.
  • Conduct outreach and service coordination activities to engage with clients and provide support.
  • Write comprehensive client social histories, social services program policies, and other relevant documents.
  • Attend community-based meetings with stakeholders and providers to represent the agency.

Requirements

  • Graduation from an accredited four-year degree-granting college or university in social work, psychology, sociology, nursing, gerontology, or a related field.
  • Two years of social services program experience interviewing clients, assessing client needs, and counseling clients.
  • Fluency in English and Spanish.

Working Conditions

This position requires the ability to work effectively with clients, colleagues, and community partners. You must be able to move intermittently throughout the day, use proper body mechanics, and occasionally lift and move objects. You must also be able to observe, listen, and communicate effectively, and function independently with flexibility and personal integrity.
