Job Overview

The Clinical Care Support Coordinator plays a crucial role in delivering high-quality clinical assistance to the adult residential and day programs at MARCH, Inc. of Manchester. This position is essential in the formulation, execution, and assessment of Positive Behavior Support Plans for all individuals receiving services, adhering to the standards set by the CT Department of Developmental Services (DDS) and the policies of MARCH, Inc. of Manchester.

Reporting Structure:

  • Supervised by the Clinical Supervisor

Supervisory Duties:

  • No supervisory responsibilities

Key Responsibilities:

  • Collaborate with Program Managers to ensure that treatment planning is centered around the individual, aimed at enhancing the quality of life for those served and the direct care professionals involved.
  • Support Program Managers in providing therapeutic programs that foster socially acceptable skills, promoting independence in self-care, daily living activities, and community engagement.
  • Conduct monthly meetings with Program Managers to evaluate the effectiveness of Positive Behavior Support Programs, ensuring that the overall effectiveness rating meets or exceeds 80%.
  • Assist in the development and execution of the Individual Planning process related to Positive Behavior Support Plans and clinical objectives, ensuring adherence to appropriate treatment protocols.
  • Compile quarterly data into a Behavior Summary Report to track progress in the Individual Planning Process.
  • Engage with the Interdisciplinary Team (IDT) in Person-Centered Individual Planning meetings at least every six months.
  • Contribute to the Program Review Committee (PRC) and Human Rights Committee (HRC) documentation, ensuring timely submission of all required materials to the CT Department of Developmental Disabilities (DDS).
  • Uphold the rights of individuals served and their families by maintaining confidentiality, adhering to the least restrictive procedures philosophy, and following ethical standards.
  • Participate in the clinical aspects of new program development and implementation.
  • Maintain compliance with all certification and training requirements established by regulatory bodies and agency policies throughout employment.
  • Exhibit proficiency in positive behavior supports, applied behavior analysis, and person-centered planning.
  • Safeguard the confidentiality of all individuals served and their families in accordance with DDS licensing regulations, MARCH, Inc. policies, and HIPAA guidelines.

  • Preferred experience working with individuals with intellectual or developmental disabilities.
