Mental Health Case Manager

Harbor is seeking a highly skilled and compassionate CPST/TBS Provider (Case Manager) to join our mental health team in Toledo. As a key member of our team, you will play a vital role in providing exceptional care and support to our clients.
Responsibilities- Documentation and Assessment: Accurately and timely document client progress, including activity notes, ISPs, and incident reports.
- Crisis Prevention and Intervention: Develop and implement strategies to prevent and respond to crisis situations.
- Client Support and Education: Provide education and consultation to clients, families, and significant others to promote healthy coping mechanisms and social support networks.
- Service Coordination: Coordinate and monitor all services identified in the client's ISP, ensuring seamless delivery of care.
- Community Resource Access: Assist clients in accessing essential community resources to support their recovery and well-being.
