Job Description: Family Care Coordinator

Estimated Salary: $60,000 - $70,000 per year

The Family Care Coordinator will play a key role in ensuring that children and families receive the support they need to thrive. This includes coordinating and facilitating meetings with caregivers, family members, natural supports, and professionals.

Key Responsibilities:

  1. Coordinate and facilitate Service Coordination Planning meetings for youth clients of the ICTS program.
  2. Identify and engage natural supports and professionals who can contribute to the child's support plan.
  3. Develop and implement individualized plans to meet the unique needs of each client.

Qualifications:

  • Bachelor's degree in social services, behavioral health, or a related subject.
  • Ability to work collaboratively with diverse groups and individuals.
  • Strong communication and interpersonal skills.
