Key Responsibilities:

  • Coordinates research data, community physicians, hospital resources into a seamless model of access and care benefiting patients, participating physicians, and family members.
  • Performs other related duties.

Qualifications:

  • Bachelor's degree in a health-related field required.
  • Master's degree in a health-related field preferred.
  • Three years of experience in patient education, nursing, planning, case management, or similar experience.
  • RN license in state of practice.
