The Clinical Social Worker at PeaceHealth provides essential support in coordinating patient care and developing treatment plans. This role comes with a competitive salary range of $44,250 - $66,370 per year.

About the Position

This position requires a Master's degree in Social Work from an accredited institution and at least 2 years of clinical experience with the designated population. LCSW or CSWA certification is also mandatory.

Responsibilities
  • Develop treatment plans in collaboration with the treatment team, considering psychiatric, chemical dependency, and psychosocial factors.
  • Ensure seamless transitions in care through effective disposition and discharge planning.
  • Maintain accurate records and stay updated on community resources to deliver high-quality care.
  • Work collaboratively with the treatment team to provide brief therapeutic interventions and coordinate community staffings.
  • Protect patient confidentiality and adhere to relevant laws and regulations.
