At SSM Health, we're committed to delivering exceptional patient care. As an RN Navigator - Patient Care Coordinator, you'll play a vital role in ensuring our patients receive the highest level of service throughout their healthcare journey.
Key Responsibilities:
- Collaborate with physicians and advanced practice providers to coordinate care and promote quality outcomes.
- Function as a liaison between management staff, administration, and other stakeholders to ensure seamless communication and coordination.
- Compile data and track outcomes to inform process improvements and drive quality care.
- Provide referrals and support for follow-up care for patients who are uninsured or underinsured.
- Utilize the nursing process to assess, plan, intervene, and evaluate patient care.
Requirements:
- Graduate of an accredited school of nursing or equivalent education.
- Three years' registered nurse experience.
Physical Requirements:
- Constant use of speech to share information through oral communication.
- Constant standing and walking.
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, reaching, and keyboard use/data entry.
Professional Licenses and Certifications:
- Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA).
- Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR).
