South Middlesex Opportu is seeking a skilled Case Manager to provide supportive services to chronically homeless individuals with relapse histories. As a key member of our team, you will be responsible for conducting initial and ongoing assessments, developing individual service plans, and providing case management services to ensure participants meet HUD requirements.
Key Responsibilities- Develop and implement individual service plans for participants, ensuring adherence to program policies and protocols.
- Conduct regular outreach and engagement with participants, including home visits and group facilitation.
- Collaborate with shelter staff, relapse prevention managers, and sober housing staff to identify new program participants.
- Maintain accurate and up-to-date records, including case notes and statistical data.
- Provide crisis intervention and support to participants as needed.
- Engage with participants to understand and address their needs, both within and outside the scope of work.
- Bachelor's degree in a related field or equivalent experience working with the target population.
- Safe and reliable transportation.
- Substance abuse experience required.
- Excellent interpersonal and organizational skills.
- Previous experience with group facilitation preferred.
This position requires travel within the state and direct or incidental contact with clients served by South Middlesex Opportu. A successful background check is required.
