Position Summary

The Community Support Specialist (CSS) plays a vital role in evaluating the psychosocial needs of patients and their families. This position involves planning, executing, and assessing care strategies while engaging with clients in their living environments. The CSS aims to enhance the quality of life and health literacy among clients. Responsibilities include coordinating care, ensuring effective communication across healthcare teams, and optimizing resource utilization.

Key Responsibilities

  • Deliver field-based case management services to designated client groups.
  • Address challenges that can reduce healthcare costs while improving overall quality of life.
  • Build and maintain positive relationships with clients, caregivers, healthcare providers, and team members.
  • Conduct assessments and develop tailored care plans.
  • Collaborate with colleagues to enhance patient outcomes.
  • Provide education to manage physical health issues.
  • Connect clients with community resources.
  • Assess dietary needs and home safety.
  • Guide safe medication practices.
  • Educate clients on health conditions and treatment options.
  • Support stress management and emotional well-being.
  • Conduct home visits to monitor client progress.
  • Ensure compliance with privacy regulations.
  • Maintain accurate documentation of care coordination in electronic medical records.

Qualifications

  • Preferred: One (1) year of experience in a similar role.
  • Proficient in Microsoft Office Suite.
  • Ability to adapt to policy changes and new responsibilities.
  • Valid driver's license with a clean driving record.
  • Knowledge of privacy regulations.
  • Bilingual skills (English/Spanish) are a plus.

Education and Certifications

  • High School Diploma or equivalent required.
  • Bachelor's Degree in Social Work (BSW) is strongly preferred.
  • Consideration for candidates with a combination of education and experience.
  • Current and valid driver's license required.
