Position Summary

The Community Support Specialist (CSS) plays a crucial role in evaluating the psychosocial needs of patients and their families. This position involves planning, executing, and assessing care strategies while engaging with clients in their residences. The CSS aims to elevate the quality of life and health literacy among clients. This role includes coordinating care, ensuring timely communication across various healthcare services, and optimizing the use of available resources.

Key Responsibilities

  • Deliver field-based case management services to designated client groups.
  • Address challenges that reduce healthcare costs while enhancing the quality of life.
  • Foster strong relationships with clients, caregivers, healthcare providers, and team members.
  • Conduct thorough assessments and develop tailored care plans.
  • Collaborate with colleagues to improve client outcomes.
  • Provide education to manage physical health concerns.
  • Connect clients with relevant community resources.
  • Assess dietary needs and home safety.
  • Guide clients on safe medication practices.
  • Educate clients about specific health conditions and treatments.
  • Support clients in stress management and emotional well-being.
  • Monitor client progress through regular home visits.
  • Adhere to HIPAA regulations and maintain confidentiality.
  • Document care coordination accurately in the electronic medical record (EMR).

Qualifications

  • Preferred: One (1) year of experience in a similar role.
  • Proficient in Microsoft Office Suite.
  • Ability to adapt to evolving policies and responsibilities.
  • Valid driver's license with a clean driving record.
  • Understanding of HIPAA regulations.
  • Bilingual (English/Spanish) candidates are preferred.

Education and Certifications

  • High School Diploma or equivalent required.
  • Bachelor's Degree in Social Work (BSW) is strongly preferred.
  • Consideration for candidates with a combination of education and experience.
  • Current and valid driver's license required.
