Position Summary
The Community Support Specialist (CSS) plays a crucial role in evaluating the psychosocial needs of patients and their families. This position involves planning, implementing, and assessing care needs while engaging with clients in their own environments. The CSS aims to enhance the quality of life and health literacy among clients, ensuring effective communication and coordination of care across various healthcare settings.
Key Responsibilities
- Deliver field-based case management services to designated client populations.
- Address issues that can reduce healthcare costs while improving clients' quality of life.
- Build and maintain positive relationships with clients, caregivers, healthcare providers, and team members.
- Conduct assessments and develop tailored care plans.
- Collaborate with peers to enhance client outcomes.
- Provide education to manage physical health concerns.
- Connect clients with community resources and services.
- Assess dietary needs and conduct home safety evaluations.
- Educate clients on safe medication practices and specific health conditions.
- Offer emotional support and stress management techniques.
- Monitor client progress through regular home visits.
- Ensure compliance with HIPAA regulations.
- Maintain accurate documentation of care coordination in the electronic medical record (EMR).
Qualifications
- Preferred: One (1) year of experience as a Community Support Specialist.
- Proficient in Microsoft Office Suite.
- Ability to adapt to changes in policies and procedures.
- Valid driver's license with a clean driving record.
- Knowledge of HIPAA regulations is essential.
- Bilingual (English/Spanish) is a plus.
Education and Certifications
- High School Diploma or equivalent is required.
- Bachelor's Degree in Social Work (BSW) is strongly preferred.
- A combination of education and experience may be considered.
- Current and valid driver's license is required.
