We are seeking a skilled Home Health Care Professional to join our team at Elliot Health. As a Home Health Care Professional, you will provide high-quality care to patients in the comfort of their own homes.

Responsibilities
  • Deliver home health care to clients/patients of all ages and families whose needs range from basic to complex and are rapidly changing.
  • Create a therapeutic environment while identifying and treating human responses to actual or potential health problems.
  • Practice under the direction of the Clinical Manager and supervision of the RN within the departmental policies and procedures.
  • Review all medical data prior to delivering patient care, document and communicate patient status and assess needs to the Home Care Nurses.
  • Participate in the development of a patient care plan which will facilitate health care, promote patient comfort and well-being, and includes patient/family.
  • Integrate problem solving methodology and quality improvement in approaching problem areas related to patient care and organizational processes.
  • Provide nursing care while promoting client/patient dignity and provides a calm, supportive and therapeutic environment for client/patients and their families.
  • Obtain prior authorizations and documents adequately to ensure prevention of write-offs.
  • Explain insurance coverage and cost of care appropriately to patients/significant others.
Requirements
  • Graduate of an accredited school of Licensed Practical Nursing required.
  • Minimum of 2 years previous clinical experience required, preferably in a home care setting.
  • Valid Driver's License required to travel throughout southern New Hampshire to provide home health care service and other job-related activities.
