Responsibilities- Assess patients' physical and psychosocial status and the home environment to identify their needs and develop a personalized care plan.
- Review medical data and communicate with healthcare professionals to ensure seamless care coordination.
- Develop and implement a patient care plan that promotes health, comfort, and well-being, and includes the patient and their family in the decision-making process.
- Communicate effectively with patients, families, and healthcare professionals to ensure that their needs are met and that they receive the best possible care.
- Integrate problem-solving methodologies and quality improvement strategies to address patient care and organizational processes.
- Provide nursing care that promotes patient dignity and creates a calm, supportive, and therapeutic environment.
- Obtain prior authorizations and document patient information to ensure accurate billing and prevent write-offs.
- Explain insurance coverage and cost of care to patients and their families to ensure transparency and understanding.
- Graduate of an accredited school of Licensed Practical Nursing required.
- Minimum of 2 years previous clinical experience required, preferably in a home care setting.
- LPN NH licensure required. Current BLS certification or must obtain within 90 days of date of hire required.
- Valid Driver's License required to travel throughout southern New Hampshire to provide home health care services.
