We are seeking a skilled and compassionate Home Health Care Nurse to join our team at Elliot Health. As a Home Health Care Nurse, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.
Key Responsibilities:- Assess patients' physical and psychosocial status and the home environment.
- Review all medical data prior to delivering patient care, document and communicate patient status and assess needs to the Home Care Nurses.
- Under direction of Home Care RN, participate in the development of a patient care plan which will facilitate health care, promote patient comfort and well-being, and includes patient/family.
- Communicate appropriate information to patients, peers, managers, physicians, department, and other agencies involved.
- Integrate problem solving methodology and quality improvement in approaching problem areas related to patient care and organizational processes.
- Provide nursing care while promoting client/patient dignity and provides a calm, supportive and therapeutic environment for client/patients and their families.
- Obtain prior authorizations and documents adequately to ensure prevention of write-offs.
- Explain insurance coverage and cost of care appropriately to patients/significant others.
- Graduate of an accredited school of Licensed Practical Nursing required.
- Minimum of 2 years previous clinical experience required, preferably in a home care setting.
- LPN NH licensure required. Current BLS certification or must obtain within 90 days of date of hire required.
- Valid Driver's License required to travel throughout southern New Hampshire to provide home health care service and other job-related activities.
