About the Role:

We are seeking a skilled and compassionate Home Health Care Professional to join our team at Elliot Health. As a Visiting Nurse, you will play a vital role in delivering high-quality care to patients in their homes, promoting independence, and improving their overall well-being.

Key Responsibilities:

  • Deliver comprehensive home health care services to patients with diverse needs, including basic to complex care.
  • Create a therapeutic and caring environment for patients and their families, promoting dignity, joy, and comfort.
  • Collaborate with interdisciplinary teams to ensure seamless care across shifts and settings.
  • Embrace innovation and inspire change to support optimal patient outcomes and a professional practice environment.
  • Practice in a manner consistent with the Elliot Health Interprofessional Practice model, the ANA's Social Policy Statement, Scope and Standards of Practice, and relevant specialty standards of practice.

Requirements:

  • Graduate of an accredited school of nursing, with a Bachelor of Science in Nursing preferred.
  • New Hampshire Registered Nurse or Compact State Registered Nurse License.
  • Ability to obtain BLS qualification during orientation period and maintain during employment.
  • Pursue professional certification upon eligibility or within three years of entering the specialty practice.
  • Valid Driver's License required to travel throughout southern New Hampshire to provide home health care services.
