About the Job:

We are seeking a skilled Social Worker to join our Home Health Care team. As a Social Worker, you will provide social work services, including crisis intervention and supportive counseling to clients/patients of all ages and their families relative to illness or injury.

Key Responsibilities:

  • Assess clients'/patients' psychosocial, environmental, financial, and functional status to coordinate appropriate plans.
  • Document social work interventions in the client's/patient's medical record pertaining to the referral source, reason for referral, issues, recommendations, and a treatment plan resulting in an established outcome.
  • Provide social work intervention to active clients/patients and family members.
  • Assist client/patient and family with difficulty in coping with residual physical disabilities and fear related to helplessness and death through clinical social work intervention.
  • Case conferences weekly with BSW for all new BSW admissions and reviews/determines appropriateness, changes needed in developing a plan of care.

Requirements:

  • Master of Science in Social Work required.
  • One (1) to three (3) years of relevant professional experience in a health care setting.
  • Valid Driver's License is required to travel throughout southern New Hampshire to provide home health care service and other job-related activities.
