The Service Coordinator plays a vital role in linking residents of affordable housing communities with essential support services that foster independence and enhance their overall well-being. This position involves providing residents with valuable information, making referrals to local organizations, and facilitating access to resources on-site. Together, we aim to cultivate safer and more nurturing communities.

Key Responsibilities:

  • Establish volunteer support initiatives in collaboration with community service organizations.
  • Guide and inform residents and their families about available services that encourage wellness and self-sufficiency.
  • Educate residents, families, and staff regarding community resources, including case management, personal assistance, meal delivery, transportation, counseling, and preventive health services.
  • Support residents in forming informal support networks with family and friends.
  • Oversee the provision of services to residents, ensuring they are timely, appropriate, and satisfactory.
  • Build and maintain relationships with local social service agencies to enhance residents' access to programs.
  • Advocate for community needs among local service providers and promote partnerships that facilitate access to necessary services.
  • Foster a sense of community among residents through organizational and facilitation skills, which may include developing social programs, resident associations, newsletters, and support groups.
  • Maintain accurate resident records and documentation within a tailored data management system.
  • Success Criteria: A genuine passion for service coupled with a strong work ethic.
Qualifications:
  • Two or more years of direct experience in supportive or disability services.
  • A Bachelor's Degree in Social Work, Gerontology, Psychology, or Counseling is preferred.
  • Proven ability to advocate, organize, direct, and problem-solve effectively.
  • Excellent communication skills with the capacity to act as a liaison between residents and management.
  • Adept at engaging with a diverse range of individuals, including residents and service providers, to assess needs and monitor service delivery.
  • Experience in developing and managing programs aimed at enhancing quality of life and cognitive engagement.
  • Skills in crisis management and case management.
  • Ability to complete required reporting activities in a timely manner.
  • Strong networking and community-building capabilities.
  • Coordinate special initiatives to promote health, wellness, and education among residents.
  • Implement mechanisms for monitoring and evaluating service effectiveness.
  • Uphold confidentiality at all times.
Work Environment:

Work is conducted in both indoor and outdoor residential settings, with regular exposure to varying temperatures, noise levels, and a diverse population.
