Key Responsibilities:
- Establish volunteer support initiatives in collaboration with community service organizations.
- Guide and inform residents and their families about available services that foster wellness and independence.
- Provide education to residents, families, and staff regarding local resources such as case management, personal assistance, transportation, and legal advocacy.
- Encourage residents to form informal support networks with each other, family, and friends.
- Oversee the delivery of services to residents, ensuring they are timely, appropriate, and satisfactory.
- Build and maintain a network of local social service agencies to facilitate residents' access to necessary programs.
- Promote awareness among local service providers about community needs and foster partnerships that enhance service access for residents.
- Utilize organizational and facilitation skills to develop a sense of community among residents and their neighbors, which may include organizing social events, creating newsletters, and establishing support groups.
- Maintain accurate resident files and documentation within a specialized data management system.
- Success Criteria: A genuine passion for service and a strong work ethic.
- Two or more years of relevant experience in supportive or disability services.
- A Bachelor's Degree in Social Work, Gerontology, Psychology, or Counseling is preferred.
- Proven ability to advocate, organize, direct, and problem-solve effectively.
- Excellent communication skills, serving as a liaison between residents and management.
- Ability to engage with a diverse range of individuals, including residents and service providers, to assess needs and monitor service delivery.
- Experience in developing and managing programs aimed at enhancing quality of life and cognitive engagement.
- Skills in crisis management and case management.
- Completion of required reporting activities as mandated.
- Networking and community-building expertise.
- Coordination of special programs focused on health, wellness, and education.
- Ability to create mechanisms for monitoring and evaluating service effectiveness.
- Commitment to maintaining confidentiality at all times.
Work is conducted in both indoor and outdoor residential settings, with regular exposure to varying temperatures, noise levels, and interactions with people and equipment.
