Job Summary

We are seeking a highly skilled and compassionate Pediatric Genetics Outpatient RN Coordinator to join our team at Yale New Haven Health. As a key member of our multidisciplinary team, you will play a vital role in ensuring seamless coordination of patient care across the continuum.

Key Responsibilities
  • Professional Practice: Lead the healthcare team in coordinating all aspects of patient care for the specific specialty.
  • Education: Provide patient and family education on medical conditions, treatment regimens, and management of conditions.
  • Multi-Disciplinary Collaboration: Function as a key member of the multi-disciplinary team coordinating all aspects of patient care across the continuum.
  • Quality Improvement: Participate in data collection related to clinical and operational efficiencies.
Qualifications
  • Education: Registered Nurse, Bachelor's Degree in Nursing preferred, Connecticut RN license.
  • Experience: At least 2-3 years RN experience in an inpatient or outpatient setting required. Relevant experience in the specialty area, pediatrics or in the Nurse Coordinator role preferred.
