Pediatric Hematology RN Coordinator- Greenwich

We are seeking a highly skilled and experienced Pediatric Hematology RN Coordinator to join our team at Yale New Haven Health. The successful candidate will be responsible for coordinating all aspects of patient care for the Pediatric Hematology specialty, ensuring seamless care across the continuum.
Key Responsibilities- Provide professional practice leadership and coordination of patient care for the Pediatric Hematology specialty.
- Develop and implement patient education programs to promote optimal health outcomes.
- Collaborate with multidisciplinary teams to ensure effective care coordination and communication.
- Participate in quality improvement initiatives to enhance clinical and operational efficiencies.
- Registered Nurse with a Bachelor's Degree in Nursing, Connecticut RN license, and 2-3 years of experience in nursing and infusion therapy.
- Excellent interpersonal and organizational skills, with the ability to assert oneself, advocate for patients, and work independently.
- Computer experience and knowledge of word processing and data entry systems.
- Ability to communicate effectively with all members of the healthcare team, patients, and families.
This position is fully funded by grants and contracts, and the successful candidate will be an integral member of our multidisciplinary team providing developmentally appropriate care to patients and families.
