We are seeking a skilled RN Clinician to join our team at Providence Infusion and Pharmacy Services. As a key member of our healthcare team, you will provide direct nursing services and case management related to infusion therapy for patients in the home, clinic, or alternate living site.
Key Responsibilities- Collaborate with patients, caregivers, physicians, and other healthcare providers to develop and implement individualized care plans.
- Provide education and support to patients and their families on infusion therapy and related care.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
- Manage and coordinate infusion therapy services, including medication administration and monitoring.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
- Education: Bachelor's degree in Nursing (BSN) or equivalent.
- License: Current RN license in the state of practice.
- Experience: At least 1 year of experience in acute care nursing, with a focus on infusion therapy or home health care.
- Skills: Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
