About the Position

Care Coordinators play a critical role in ensuring that individuals receive the necessary support and resources to achieve their goals. As a Justice-Involved Care Coordinator, you will work closely with clients, community agencies, and healthcare providers to coordinate comprehensive care plans.

Salary: $42,000-$45,000 based on qualifications and experience.
