About the Role

We are seeking a highly skilled RN Care Manager Ambulatory to join our team at Intermountain Healthcare. As a key member of our care management team, you will play a critical role in improving the health outcomes of our patients with chronic conditions or complex needs.

Key Responsibilities
  • Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans that meet the unique needs of our patients.
  • Conduct thorough assessments of patients' medical, functional, and social conditions to identify areas for improvement and develop strategies for addressing these needs.
  • Work closely with patients and their families to educate them on self-management techniques and provide support and resources to help them achieve their health goals.
  • Coordinate discharge planning and ensure a smooth transition of care for patients being discharged from the hospital or other healthcare settings.
  • Identify and connect patients with community resources and support systems to help them access the care and services they need.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed to ensure optimal outcomes.
Requirements
  • Graduate of an accredited school of nursing with a current valid State RN License.
  • Three (3) years of experience as a Registered Nurse in an acute care setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
Preferred Qualifications
  • BSN degree.
  • Five (5) years of experience as a Registered Nurse in an acute care setting.
Physical Requirements
  • Ongoing need for employees to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy.
