- Evaluates clients and families to determine the need for specific social work services and collaborates with the Licensed Social Worker to develop a care plan.
- Coordinates with other agencies regarding the treatment plan or care strategy, ensuring follow-up on connections made between patients and other service providers to maintain continuity of care.
- Assists staff, patients, families, and other caregivers with the application process for third-party reimbursements, entitlements, and community resources.
- Conducts outreach to community organizations to establish collaborative relationships and facilitate access to additional services. Informs other agencies about the program and referral process.
- Keeps accurate records and statistics in line with program requirements. Clearly documents planned and delivered services, maintaining written and verbal communication in accordance with VNS Health policy.
- Participates in interdisciplinary team meetings and case discussions, working collaboratively with the team to ensure a continuum of quality in-home services for families.
- Travels to patients' homes for visits as determined by the care plan or as directed by leadership.
- Attends required in-service education programs and other training sessions as mandated by VNS Health.
- Takes responsibility for ongoing professional development.
- For Behavioral Health only:
- Assists in monitoring the impact of the care plan on the patient and family.
- Arranges transportation and/or appointments for patients at clinics, programs, and offices.
Licenses and Certifications:
- Valid driver's license or NYS Non-Driver photo ID card, as determined by operational/regional needs. Required
- Associate's Degree in Social Work, Psychology, or a related human services field or
- Four years of full-time paid experience in social work within a social agency adhering to acceptable standards or an equivalent combination of education and experience Required
- Minimum of one year of social work experience in a mental health, healthcare, or human services environment Required
- Bilingual skills may be required based on operational needs.
- This position may necessitate travel between regions.
