Job Summary:

YAI is seeking a dedicated and compassionate Community Habilitation Specialist to join our team in NYC. As a Community Habilitation Specialist, you will play a vital role in providing one-on-one support to individuals with intellectual and developmental disabilities, empowering them to achieve greater independence and community integration.

Key Responsibilities:

  • Provide individualized support to people with intellectual and developmental disabilities in community and home settings.
  • Teach various life skills, promote community integration, and ensure the health, wellness, and safety of the individuals we support.
  • Collaborate with the team to develop and implement person-centered plans, ensuring compliance and billing requirements are met.
  • Participate in mandatory trainings and apply skills to provide high-quality support.
  • Meet and demonstrate OPWDD Core Competencies.
  • Transport and escort individuals on recreational activities, medical appointments, and home visits.
  • Build positive relationships and maintain a therapeutic environment through various methods.

Requirements:

  • High School Diploma or GED preferred; some college is a plus.
  • Work or life experience within the intellectual/developmental disabilities field is a plus.
  • Basic written and verbal communication skills.
  • Basic computer knowledge.
  • Valid Driver's License may be required depending on the position.
  • Tuberculosis test mandatory for most positions.

Compensation:
