Responsibilities:
Enhabit Home Health & Hospice is seeking a RN, LVN, PT, PTA, OT, or MSW to join our team as a Patient Care Transition Specialist.
Key Responsibilities include:
Assist patients in navigating the post-acute care process.
The Admissions Coordinator will evaluate, plan, implement, coordinate, monitor, and assess options and services with the primary objective of ensuring a safe transition from acute care to home for home health or hospice services.
Utilize evidence-based clinical guidelines and preventative protocols in the development of patient-centered transition plans, promoting quality and efficiency in post-acute care delivery.
Represent Enhabit in transitional care initiatives and strategic partnerships with health systems, hospitals, inpatient facilities, and physician groups.
Oversee the execution of transitional care services through continuous quality assurance visits with referral sources.
Meet and/or exceed referral and admission targets.
Act as a clinical liaison responsible for the care transitions program admission activities within the designated territory, positively influencing patient outcomes and referral source satisfaction.
Qualifications:
The ideal candidate for this position will be a RN, LVN, PT, PTA, OT, or MSW who is goal-oriented, sales-driven, and possesses prior experience in home health or hospice.
Must be a graduate of an accredited nursing or therapy program and hold a valid license in the state of employment.
Excellent communication skills and the ability to build relationships, engage effectively with diverse individuals, and possess strong presentation abilities are essential.
