Clinical Pharmacy Specialist, Day Shift, Transition of Care

As the Clinical Pharmacy Specialist in Transition of Care you will:
• Evaluate the appropriateness and effectiveness of the patient's medications.
• Follow the patient's progress to determine the effects of the patient's medications on his or her health.
• Consult with the patient's physicians and other health care providers in selecting the medication therapy that best meets the patient's needs and contributes effectively to the overall therapy goals.
• Review and verify physician orders for appropriateness
• Participate in Patient care rounds
• Counsel and educate patients about their medications.
• Serve as a preceptor for Pharmacy resident and students
• Educate pharmacists, nurses and physicians
• Participate in hospital committees
• Participate in Pharmacy Staff monthly meetings and weekly huddles
• Prospective medication discharge review, medication discharge counseling, and follow-up to ensure medication compliance
• Work in conjunction with the Care Navigation team to ensure availability and affordability of prescribed medication and offer alternative treatments if protentional issue are identified.
Qualifications include:
• Required: Graduation from an ACPE accredited pharmacy school, with • PharmD degree
• Required: ASHP accredited PGY-2 Residency, or PGY-1 residency with •
Minimum of 3 years of clinical pharmacy experience in area of specialty
• Required: Registered Pharmacist with the Maryland Board of Pharmacy
• Preferred (required within one year of employment): Board Certification in area of specialty
• Preferred: ACLS Certification
• Preferred: Member of American Society of Health-system Pharmacists (ASHP), and/or Maryland
Work Schedule:
9:00 am - 5:30 pm Monday-Friday
