Job Description

We are seeking a qualified Children's Case Manager to join our team in Apache Junction. As a key member of our team, you will provide direct service crisis intervention, support, transportation, and education to families and children, working independently or collaboratively with other in-home client services. Your primary responsibilities will include conducting family needs assessments, making referrals to collaborating partners and community services as needed, and ensuring continuity of care through comprehensive services and coordinated communication.

What You'll Do

  1. Assess family needs, identify gaps in service, and make referrals to collaborating partners and community services.
  2. Develop and implement individualized service plans to address the unique needs of each family.
  3. Provide direct service crisis intervention, support, transportation, and education to families and children.
  4. Cultivate positive relationships with clients, colleagues, and community partners to ensure effective collaboration and coordination of services.
