Job Title: Case Manager I - Overnight

At Heartland Family Service, we are seeking a skilled and compassionate Case Manager I - Overnight to join our team. As a key member of our overnight staff, you will provide facility oversight and assist in duties to ensure the day-to-day operations of our program.

Responsibilities:
  • Develop supportive relationships with clients and respond to their needs during your shift
  • Document client activity per established record-keeping procedures
  • Monitor client self-administration of medication and follow procedures for recording
  • Execute full medication counts on a nightly basis
  • Answer crisis care referral calls, assess individuals, and provide appropriate and accurate application verdicts while maintaining strict confidentiality
  • Complete enrollment and intake on eligible clients
  • Conduct nightly housing lead searches
  • Psycho-educate clients on life, social, and coping skills
  • Provide problem-solving and crisis intervention services to empower clients to develop self-sufficiency
  • Utilize trauma-informed care, person-centered, and culturally competent principles in providing services to clients
  • Demonstrate knowledge of current community and HFS programs and resources to meet client needs
  • Complete monthly client record reviews
  • Conduct client room searches per program protocol
  • Help ensure room readiness for new clients
  • Orient clients to Bridges protocols and address client non-compliance as necessary to create a safe and secure environment for all residents and staff
  • Attend All Staff meetings as scheduled
  • Responsible for maintenance and upkeep of the facility
  • Able to work both independently and as a part of a team
  • Always display a courteous and caring attitude to clients, volunteers, and visitors
  • Abide by all specific program and Agency procedures, policies, and requirements
  • Cooperate and collaborate with program area staff, volunteers, and other Agency staff
  • Develop personal and program-related skills through participation in internal and external training opportunities
  • Create, maintain, and share a dynamic self-care plan
Requirements:
  • High School diploma or associate degree
  • Two years of experience in human services
  • Valid driver's license and acceptable driving record
