Job Title: Lead Case Manager

At Heartland Family Service, we are seeking a highly skilled and compassionate Lead Case Manager to join our team. As a key member of our crisis stabilization team, you will play a vital role in providing intensive supervision, de-escalation, and case management services to individuals in crisis.

Job Summary:

The Lead Case Manager will serve as the program's lead crisis stabilization specialist, working with individuals who require a safe and secure environment to stabilize from a crisis. This role requires strong decision-making, problem-solving, and interpersonal skills, as well as the ability to work collaboratively with clients, staff, and community resources.

Responsibilities:
  • Provide intensive supervision, de-escalation, and case management services to individuals in crisis
  • Coordinate supportive services to meet the needs of clients, including connecting them with medical, therapeutic, or other providers
  • Develop and implement individualized service plans to promote client stability and self-sufficiency
  • Collaborate with program supervisors to train staff on crisis stabilization services and protocols
  • Ensure accurate and timely documentation of client activity, service plan progress, and needs
  • Provide problem-solving and crisis intervention services to empower clients to develop self-sufficiency
  • Screen for mental health and substance abuse disorders and provide linkages to appropriate services
  • Assist clients in locating, obtaining, and retaining suitable housing through housing search and placement services
  • Monitor client self-administration of medication and follow procedures for recording
  • Willingness and ability to obtain/maintain certification to administer non-parenteral medications in the state of Iowa
  • Accurately record client referrals and service outcomes on designated databases
  • Submit prior authorization forms to client managed care organizations to secure funding for CSRS services
  • Psycho-educate clients on life, social, and coping skills
  • Demonstrate knowledge of current community and HFS programs and resources to meet client needs
  • Utilize trauma-informed care, person-centered, and culturally competent principles in providing services to clients
  • Evaluate efficiency and effectiveness of community service providers on an ongoing basis to ensure clients are receiving quality care
  • Orient clients to Bridges protocols and address client non-compliance as necessary to create a safe and secure environment for all residents and staff
  • Conduct client room checks and searches per program protocol
  • Display a courteous and caring attitude to clients, staff, and visitors
  • Abide by all specific program and Agency procedures, policies, and requirements
  • Cooperate and collaborate with program area staff, volunteers, and other Agency staff
  • Develop personal and program-related skills through participation in internal and external training opportunities
  • Strive to make connections between the agency and the larger community to contribute to the agency's ongoing fundraising and friend-raising efforts
Requirements:
  • Bachelor's Degree in a human services field (including but not limited to, psychology, social work, mental health counseling, marriage and family therapy, or a related field)
  • Three years of experience in behavioral or mental health
  • Experience working with adults with serious mental illness or substance use preferred
  • Valid driver's license and acceptable driving record

We offer a competitive salary range of $21.79 to $25.98 per hour, depending on experience, as well as a comprehensive benefits package and opportunities for professional growth and development.
