Job Summary

We are seeking a skilled Social Worker to join our team. The successful candidate will be responsible for providing social work services to patients, addressing social determinants of health, and connecting patients to community resources.

Key Responsibilities
  • Conduct initial assessments for social determinants of health
  • Provide direct services or connect patients to programs to address SDoHs
  • Assist individuals and/or families in finding satisfactory ways of coping with and understanding the nature and extent of problems in social situations and relationships
  • Educate patients on available community resources and how to access them
  • Serve as the main point of contact for patients who present with any type of abuse or neglect
  • Provide case management to referred patients
  • Conduct outreach and networking as necessary
  • Coordinate referrals with community organizations
  • Maintain and build relationships with community resource agencies
Requirements
  • Bachelor's degree in Social Work or similar field
  • Current, valid certification or ability to obtain licensure as a Social Worker in the State of Iowa
  • Minimum of two years of experience providing case management to individuals, families, and groups preferred
