Community Services Coordinator

About the Role:
St. Catherine's Center for Children is seeking a skilled Community Based Services Provider to join our team. As a Community Based Services Provider, you will play a vital role in providing behavioral health support services to youth within our Home and Community Based Services (HCBS) program.
Key Responsibilities:
- Engage directly with youth and families to provide individualized teaching, coaching, and rehabilitative counseling to build skills and capacities.
- Provide services as identified on the youth's Treatment/Service Plan, following specific frequency, scope, and duration.
- Assist families in understanding and supporting their child's mental health and developmental needs.
- Communicate effectively with other providers and family members to advocate for the youth's needs and well-being.
- Document services accurately and timely in an electronic health record.
- Travel locally to provide services in the youth's home, school, or community settings.
- Maintain a positive, strength-based, and person-centered approach at all times.
Requirements:
- Bachelor's Degree in social work, psychology, or a related human services field, plus two years of applicable experience in children's mental health, addiction, foster care, or a related field.
- Valid New York State Driver's License, with a reliable vehicle and proof of liability insurance.
- Ability to safely transport young children, including lifting and carrying infants and toddlers.
- Flexibility to accommodate client schedules, including some early evening hours.
- Excellent interpersonal and communication skills, with a strong attention to detail and writing aptitude.
- Ability to pass required pre-employment background checks, including fingerprinting.
